The Vietnamese Health Center is designed by and for the Vietnamese Community.  We are devoted to providing access to health and social services.

        A program of Catholic Community Service and the Archbishop's Commission on Community Health

        A WEEKLY CLINIC TO:
        Example Image   Assess medical history, provide physical examinations and
             review medications in examinations and review medications in
             order to establish a baseline health status;
        Example Image   Assist with follow-up services and with finding a "medical
             home" in established health agencies;
        Example Image   Supply medications and treatments for minor illnesses and
             conditions;
        Example Image   Educate about personal health and health care resources. 

        OUR HEALTH OUTREACH PROVIDES:
        Example Image   Assistance in expediting visits in hospitals, clinics, or doctor's
             office for conditions requiring more extensive health care.
        Example Image   Interpretation for clients during visits to providers.
        Example Image   Home visits from Friends of Families, the community
             outreach team.
        Example Image   Individual and community education on health issues.
        Example Image   Assistance with special health and social needs
        Example Image   Care coordination.

        MISSION
        The Vietnamese Health Center is a community based volunteer service which assists Vietnamese families to access appropriate and culturally sensitive health care and other social services.

        This transitional program facilitates linkages with existing health and social support systems by providing coordination of services, outreach, and community health education.

        GOALS AND OBJECTIVES:
        Example Image   Identify the medical and social needs of individuals and
             families.

        Example Image   Follow-up on needs identified during clinic and office contacts.
        Example Image   Coordinate care for individuals and families.
        Example Image   Educate people about healthy life-styles, primary care needs,
             and use of health and social support systems.
        Example Image   Identify at-risk populations and design programs to address
             their needs.
