Outreach ServicesPrior to the event, mobile outreach teams distribute fliers in areas frequented by homeless persons. Then, on the day of the event, the mobile outreach team provides transportation for homeless persons to the facility. Client Support ServicesUpon arrival, clients are directed to a secure area where they can store their belongings. Clients then proceed to the Welcome/Registration area, where Registration teams conduct an initial assessment to determine what services the clients need. Following the assessment, clients are referred to one of three linkages' stations where they may receive the following services: Health/Medical: medical screenings; HIV testing/counseling; TB screening/counseling ; medication assistance; vision screening; substance abuse counseling; dental care Mainstream Resources: Social Security; food stamps; Veteran's benefits; employment services; legal services Housing Services: Shelter + Care; Permanent housing for persons with disabilities; housing financial assistance; Section 8 programs/section 202 programs Food DistributionThroughout the day, clients can come to the Connect Café area to receive a boxed lunch. The Connect Café operates like a restaurant with volunteers who serve as hosts, waiters, and bussers. Recreational ActivitiesThe recreational area provides a range of activities for clients as they wait to assess the various services offered. Some of these activities include: book giveaways, back massages, and access to cell phones to make long distance phone calls. Discharge/Data EntryAs clients leave, they are asked to fill out an exit survey. They will also receive a gift bag filled with personal hygiene items as well as some personal clothing items (hats and gloves). |
