Schedule an Appointment
To schedule an appointment, you must:
- Know your street & ward number
- Receive services in your assigned area/location
- Be a St. Louis City resident.
Things to bring to your appointment:
- Utility Bills (gas or electric), if applicable
- Income Verification (pay stubs; TANF/Unemployment/disability letters, etc.)
- Assets (bank account statements, etc.)
- Photo ID
- Verification of Last Address (lease, bills, school records, etc.)
- Proof of Housing Crisis (letter from shelter case worker, eviction notice, hotel receipts, etc.)
- Social Security Card
- DD 214.
