Live Like Your Life Depends On It

Master Trainers Tammy Hamilton, RD, LD, Health Care Coordinator – St. Louis Area Agency on Aging, and Orvie Prewitt ,Program Coordinator – KC Regional Arthritis Center, graduated their third class of Chronic Disease Self-Management Program Leaders. Ten enthusiastic new leaders from across the Eastern Region completed the four-day training course on September 18, 2009. The Chronic Disease Self-Management Program (CDSMP), developed at Stanford University, is called “Live Like Your Life Depends On It”.

The CDSMP “Live Like Your Life Depends On It” is supported by the Missouri Department of Health and Senior cervices Heart Disease and Stroke Prevention Program, the CDC Heart Disease and Stroke Grant, and the Missouri Regional Arthritis Program.

This group of new CDSMP Leaders will facilitate, in their community, a six-week workshop that provides tools for living a healthy life with chronic health conditions, including diabetes, arthritis, asthma and heart disease. Through weekly sessions, the workshop provides support for continuing normal daily activities and dealing with the emotions that chronic conditions may bring about.
