Open Enrollment For Dental Insurance
Information regarding Dental Source and United
Concordia will be distributed with paychecks on Friday, May 2.
The deadline for submitting forms to the Department of Personnel,
Employee Benefits Section, 1114 Market, Suite 900, is 5 p.m.,
Friday, May 23. Coverage is effective June 22 and the first payroll
deduction will begin July 11.
There are no changes in the benefits provided
by Dental Source this year. United Concordia is enhancing benefits
this year. Please review the open enrollment material for information.
You do not have to complete an enrollment form if you do not want
to make changes. The premiums for United Concordia will not increase
this year. There is a slight increase in the Dental Source Free
Access Plan but no increase in the E Plan. Following are the bi-weekly
premiums for Dental Source and United Concordia:
