UnitedHealthcare Laboratory Change

Effective May 15, 2008, LabOne will no longer be a UnitedHealthcare participating provider. Please do not confuse this with LabCorp, which is a UnitedHealthcare provider.

If you receive laboratory services from a non-participating lab, including LabOne, on or after May 15, you may be charged the higher out-of-network cost which consists of the deductible and co-insurance instead of the lower co-payment. Your out-of-network costs could be up to 7 times greater than using a participating in-network lab.

Participating physicians have been notified of this change and have received a list of participating laboratories. Please discuss this with your physician before receiving any laboratory services. Following is a short list of participating labs: AmeriPath; Berkeley Heart Lab; Dianon Systems; Esoterix, Genzyme Gynetics; Home Healthcare Laboratory of America; Horizon Molecular Laboratory; Laboratory Corporation of America (LabCorp) and Myriad Genetics Laboratories.

Go to: www.UnitedHealthcareOnline.com for additional participating labs in this area. Select Physician Directory and then General Physician Directory, search for Hospital or Other Facility, search for laboratory.

You may also contact UnitedHealthcare at 1-800-741-8786 if you have any questions regarding participating laboratories.
